Establish a New Customer Account |
Our new account set-up procedure is outlined below for your convenience:
- Complete our electronic credit application .
- Print your completed application and sign where indicated on page 5.
- Fax your completed and signed application to our International Customer Service Department at 01-239-687-1285
One of our International Customer Service Representatives will contact you within 24 hours to confirm the receipt of your application.

Payment Options
CHAD Therapeutics offers three easy payment methods:
- Credit card, including Visa, MasterCard and American Express
- Line of credit
- Wire transfer
Payment Terms
A line of credit will be extended to firms showing a favorable credit rating, or to those firms that furnish three trade references, which provide favorable credit information. If we cannot establish a favorable credit determination, our terms require credit card payment or wire transfer before shipment. If terms are granted, your account will be established with net 60-day terms.
Past Due Accounts
All accounts over sixty days are subject to an interest and service charge of 1.5% per month. This represents an annual percentage rate of 18% per year. Any customer with an unpaid invoice after 60 days will not be eligible to receive any new shipments until the account is brought current.
