Request a Return Material Authorization Number |
Whether you have a unit in need of repair, or need to return a product, you must request a Return Material Authorization Number (RMA).
You can request an RMA in one of two ways:
- Call 239-687-1280 ext 238 or 1-800-423-8870 ext. 2388 to speak with an International Customer Service Representative.
- Complete our online RMA/Warranty Check Request Form. One of our International Customer Service Representatives will respond to your request within 24 hours.

Product Repairs
If your unit is still within its 2-year warranty, it will be repaired at no charge, unless it has been damaged through abuse, negligence, unauthorized repair or accident, in which case a repair fee will apply.
Returned Product
Products in good working order may be returned to CHAD Therapeutics for a 20% restocking fee if the following conditions are met:
- The product remains unopened.
- The product is returned within 45 days of the date of shipment.
