Establish a New Customer Account |
Opening an account with CHAD Therapeutics is fast and easy:
- Complete our electronic credit application. Please note that as a matter of company policy, some organizations will not give references. A list of these companies can be found here. Please do not list any of these companies as credit references on your application.
- Print your completed application and sign where indicated.
- Fax your completed and signed application to our Customer Service Department at 239-687-1285 along with a copy of your resale certificate.
- Take advantage of our credit card payment option if needed. To expedite the processing of your first order, you can use MasterCard, Visa, or American Express. You may also request a COD shipment (standard COD charges apply).

CHAD Therapeutics offers three easy payment methods:
- Credit card, including Visa, MasterCard and American Express
- COD shipments
- Line of credit
Payment Terms
A line of credit will be extended to firms showing a favorable TRW rating or to those firms that furnish a bank reference and three trade references, which provide favorable credit information. If we cannot establish a favorable credit determination, our terms require credit card payment, cash on delivery (COD), or check before shipment. If terms are granted, your account will be set up as 2% 10 net 30 days. (Note: if the cash discount is not taken upon initial payment of the invoice, then it becomes null and void and may not be applied to future invoices).
Past Due Accounts
All accounts over sixty days are subject to an interest and service charge of 1.5% per month. This represents an annual percentage rate of 18% per year. Any customer with an unpaid invoice after 60 days will not be eligible to receive any new shipments until the account is brought current.
